Prior to your appointment, you can download and fill out our medical forms, which will make your check in process faster and easier. Please remember to bring a list of the medications you are currently taking, any recent lab work, and your referral from your primary care physician if required by your insurance plan.
Our goal is to schedule appointments so that your needs and the needs of all of our patients are met with minimal waiting time. Occasionally, delays are unavoidable. We value your time and are committed to avoiding any unnecessary delays.
You are the only one scheduled for your appointment time. If you are unable to keep your scheduled appointment, please give at least 24 hours notice so that we may offer your appointment time to another patient. Failure to cancel or not show up for your scheduled appointment will result in a $20 fee. You will be personally responsible for this charge.
If you are a healthy patient with no symptoms or history of gastrointestinal disease, you can schedule a colonoscopy without having a preliminary office visit. You can learn more about our open access screening by contacting us at 1-888-692-6566 (MY COLON).
If you have an emergency and need to see the physician immediately, contact our office and we will help you. If your emergency is after business hours, call (678) 741-5000 and you will be connected to our after hours answering service. If you are experiencing a life threatening emergency, please call 911, or go directly to the nearest emergency department.
Our telephone triage nurses are available to answer your questions about your health. If you have undergone a test, please allow 10 days for the nurse to contact you about any test results. If it has been more than 10 days, feel free to call. At times it may be necessary for you to return to the office to discuss your care with one of our nurses or with one of our physicians.
Make sure to review your insurance plan benefits as they pertain to referrals. If your provider requires a referral, it’s your responsibility to obtain one from your primary care physician prior to your appointment.
